Health conditions and contraindications
- New clients or those with injuries, health conditions or some medications will be required to complete a triage before massage or complete a PAR-Q before exercise to determine if they can be massaged or participate in exercise.
- Contraindications: Please inform me of all health conditions, medications or if you fall into any of the following categories:
– Unstable BP.
– Diagnosed with cancer within the past five years or currently receiving treatment for cancer.
– Have COVID-19 or experiencing long-term Covid-19 symptoms or post COVID-19 circulatory complications (deep vein thrombosis, DVT, stroke symptoms or pulmonary embolism).
– Pregnant or recently given birth.
– Diagnosed with any heart or respiratory conditions.
– Have had a Stroke within the last 6 months.
– Unstable Diabetes.
– Any undiagnosed health issues including; lumps, tumours, skin conditions, pain or discomfort, dizziness, fainting, bleeding, bruises.
– Long term steroid use.
Payment terms
Fees are as stated on this website but are subject to change. Clients will be given advance notice of price changes via this website or other communication methods such as email, social media or direct contact.
Clients are required to pay in advance for sessions via bank transfer within 24 hours of booking otherwise your appointment slot will be cancelled. If you are unable to access online banking please contact me to discuss other arrangements.
Package deals
Package deals at a discounted rate are offered for 8 & 12 sessions. All sessions are to be paid in advance before the first session with all bookings used within 3 months of the first session date. Refunds are at my discretion and may be issued less any sessions taken with supporting evidence from a GP or consultant that confirms either; a newly diagnosed health condition that was diagnosed after your first session or; a current health condition that was listed on the clients consultation form at sign up that has either; deemed massage or exercise unsuitable or; is a current contraindication for massage or exercise.
Cancellation Policy
I require a minimum of 24 hours notice for reschedule requests and cancellations. Appointment reschedules and cancellations made with less than 24 hours notice will forfeit 50% of the session fee. Same day cancellations, reschedules or non attendance will be charged 100% of the session fee. Any outstanding balance needs to be paid before further bookings are taken. If you have a long-term health condition that may deteriorate at short notice which was included on your consultation form please contact me ASAP to discuss your booking.
Consent
Mobile with Michelle requires the client to consent to the processing of their client data and treatment. Your signature on the online consent form confirms that you have read and understood our business policy and terms and conditions, and that you:
- Consent to information including medical details being processed for the purposes of treatments as a private client.
- Consent to treatment or exercise by Mobile with Michelle
- Consent to having details, which are relevant to the your conditions and for which you are seeking treatment or exercise to be shared with your GP/consultant or your referring medical agency on request.
- Understand that because massage therapy work involves maintained touch and close physical proximity over an extended period there may be an elevated risk of disease transmission including COVID-19. All precautions will be put in place to control and reduce the risk of transmission.
Confidentiality
All client information and clinical records will be safeguarded by Mobile with Michelle and remain confidential. Unless required to do so by law, we will not share, sell, or distribute any of the information you provide to us without your consent.
Consultations and client treatment details will not be discussed with anyone other than the client, unless they have given consent for your information to be shared with a healthcare or fitness professional.
Your data is stored online. To prevent unauthorised access or disclosure, we employ security measures to protect your information from access by unauthorised persons and against unlawful processing, accidental loss, destruction, and damage. We will retain your information for a reasonable period or as the law requires.
Data Protection
Mobile with Michelle strictly abides by and adheres to the GDPR 2018, which sets rules for the processing of personal information. It applies to both manual and computer records.
Clients have the right to access information held about them either electronically or in paper format. Clients who wish to obtain this information will need to contact Mobile with Michelle by email at info@mobilewithmichelle to sign a Release of Medical Records Form.
Disclaimer
Mobile with Michelle will not diagnose illness or health conditions, prescribe medication or suggest medical procedures unless they are qualified to do so. You may be recommended for referral to another health care professional such as a physiotherapist, osteopath or chiropractor or be asked to gain medical clearance from a GP or consultant before any booking takes place if Mobile with Michelle feels that you have a health issue or injury that is beyond their scope of expertise, qualifications or knowledge.
Please disclose all health information as requested at the initial consultation and keep Mobile with Michelle updated and informed of any changes to your health status. This includes; all medical conditions, injuries, allergies, referrals for any investigative procedures and medication you are taking or have been newly prescribed whilst being treated. If you disclose any new health information at the start or during your session your session may be terminated with 100% payment forfeited for that session until further advice or medical clearance is received.
Before your session you are required to sign the informed consent and disclaimer forms to confirm that you have read, understand and adhere to the above.
